Infection can be painful.
Accurately describing this pain, so that physicians can provide appropriate treatment, can be problematic in some cultures and communities.
The most common descriptions include "p***ing razor blades", or "broken glass", or "red hot pins".

They enabled women to point to them, without having to say the words, so that they could seek suitable treatment.
